Important:  I understand and agree to the following participation terms:

1) I understand that my child's participation in CYM sports can subject him/her to the possibility of injury. I hereby release, discharge and/or otherwise indemnify IHM Parish, including, but not limited to IHM Parish, School, IHM Athletic Council, its officers, employees and coaches , against any claim by or on behalf of the registrant as a result of the registrant's participation in IHM's sports programs. In the event of an emergency which requires medical attention; I grant the IHM coach or representative present, my permission to seek appropriate care, including the services of an ambulance or admission to a hospital, if necessary. I release the IHM coach, representative and IHM Athletic Association from any liability for seeking such medical care.

2) I understand that in order to create teams according to CYM divisional alignment, placement on individual IHM athletic teams is based primarily on each participant's ability. I also understand that certain judgements will be made relative to each participant's ability level and I hereby agree to abide by whatever team determination that is made for the above listed participant.  Once teams are selected no refunds of the registration fee will be issued

3) I understand that by registering my child for participation in IHM's athletic program, I have read and will abide by the Parent's Code of Conduct. I understand that any incidents involving violation of the code or incidents of poor sportsmanship / unruly conduct will result in disciplinary action to be taken by the IHM Athletic Association, at it's sole discretion.
